Changing your contact details

To update details like your name, address, phone numbers or email address:

• complete a change of contact details form

If you have moved to a new address which is outside of our catchment area you may still be able to stay registered with our practice, but you will no longer be eligible for home visits.

• Check you are in our catchment area

You will need to show us proof that you have changed your name or date of birth. This could be a marriage or civil partnership certificate, deed poll or birth certificate.
