Register with the practice

Practice Area

We have an open list and welcome requests for registration from patients living in or moving to the practice area, which covers: Tarporley, Eaton, Utkinton, Little Budworth, Alpraham, Bunbury, Huxley, Tarvin, Kelsall, Ashton Hayes, Mouldsworth, Delamere and Oakmere.

To Register

Please choose one of the following options:

  • Complete an online registration
  • Complete a paper version - this will need to be returned to us with proof of ID (please see section below)
  • Collect a paper copy from reception

A registration form will need to be completed for each member of your family that wishes to register with the practice.

ID and proof of address

You will need to provide proof of identification and address when handing in paper copies of your registration form. Please provide one of the following documents from each of the lists:

Proof of Identity:

Passport, driving licence, birth certificate, marriage certificate, medical card.

Proof of Address:

Utility bill, bank/building society statement, payslip (with address), external correspondence (solicitors/estate agent, showing name and address).

Accessing someone else’s information

As a family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us. Please note we do not set up proxy access for children under the age of 18.

To request proxy access, please collect a proxy access form from reception.

Linked profiles in your NHS account

Once proxy access is set up, you can access the other person’s profile in your NHS account, using the NHS App or website.

The NHS website has information about using linked profiles to access services for someone else.

Accountable GP

From 1st April 2015, practices are required to allocate all patients (including children) with a named accountable GP.

The named GP is responsible for the coordination and delivery of all appropriate services, where required based on clinical judgement, to each of their patients.

  • Patients do not need to see their named GP when they book an appointment with the practice.
  • Patients are entitled to choose to see any GP or nurse in the practice.
