As a family member or carer, you may be able to access services for someone else. We call this having proxy access. We can set this up for you if you are both registered with us. Please note we do not set up proxy access for patients under the age of 18.
To request proxy access you will need to complete a form, this can be downloaded below or you can collect from reception. The completed form will need to be returned to us in person with 2 forms of ID.
